I don’t have a way to upload my insurance** card or my prescription. Can I still use your service?

Absolutely! There are two additional ways to provide us with that information. First, we have a secure physician’s portal that can be used by your doctor’s office. There, they can complete a referral form and provide your insurance information or, you can fax us the information at (352) 513-2549. Be sure to include a copy of both sides of your insurance** card.
